To view your health records, request labs, request certain medication refills, send us messages and more please log-in to our patient portal https://myhealthrecord.com. You can also e-mail us directly from our "Contact" page on this website.

Should circumstances lead you to receiving care at another facility, please notify us. With your written permission we can request records from outside facilities. You may complete the record release form on your own and provided it to other facilities as needed.  

Our normal business hours are Monday through Friday 8am-5pm excluding 12-1pm. We are also open select Saturdays 8am-12 noon. That does not mean that we are not available should you have an urgent need. You are always welcome to call us during business hours, and if available, your provider will get in touch with you as soon as they are able. To reach Ann Dr. Odocha's medical assistant call 352-371-3212 and press 4 to speak with her or leave a message..
