We are also open select Saturdays 8am-12 noon. That does not mean that we are not available should you have an urgent need. You are always welcome to call us during business hours, and if available, your provider will get in touch with you as soon as they are able. While logged into your https://myhealthrecord.com account you can also request to see your labs, records and much more. There is always a physician on call after hours to help you in the case of emergencies by calling our office and following the prompts.  Should you need care after hours, we have arrangements with Emergency Physicians Medical Center located on Tower Road and 24th Ave to provide your after-hour care.

Should circumstances lead you to receiving care at another facility, please notify us. With your written permission we can request records from outside facilities. You may complete the record release form on your own and provided it to other facilities as needed.  The best way to ascertain the status of your health is with certain data. For persons with diabetes and/or hypertension, it is always a good idea to keep a log book when you check your blood sugar or blood pressure and bring it with you to your appointments.
