PREPARE FOR YOUR APPOINTMENT

Welcome to Primary Care Institute, your Patient Centered Medical Home. We are glad you have chosen our office for your medical and wellness needs. The care provided here is team-based with you, the patient, at the center. 

On your first visit:

1. Please arrive 30 minutes early to your appointment.
2. Bring a picture I.D. and a copy of your insurance card.
3. Provide us with a list of all medications you are currently taking (bring bottles if available) and any medical records on-hand.
4. All co-pay, deductible and/or coinsurance are due at the time of service.
5. It is the patient’s responsibility to know his/her insurance benefits. We recommend that you call your insurance company to verify your co-pay, deductible and/or coinsurance prior to your visit.
6. Please provide us with your email address for access to our patient portal. You will be able to access your records, message office staff including your provider and many other actions.
7. To better serve all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies sometimes arise. The office reserves the right to charge patients a fee for no-show or cancellation outside 24 hours prior notice.
8. At your first visit you will be guided in the selection of a preferred provider and nurse practitioner team. We will do our best to schedule your appointments with your selected provider, but on occasion we may schedule your appointment with another member of the practice team.
